We will need the following information:
(If you want to remove any words or numbers please state that in the notes)
(If you want the shirt to be the colors in the picture please state that in the notes)

- Your mascot
- Your name for sleeve (one sleeve only)
- Your child's number (one number)
- Your team colors (Two colors only/color chart in photos)
