(If you want to remove any words or numbers please state that in the notes)
(If you want the shirt to be the colors in the picture please state that in the notes)

- Grandparent name (Mimi, Nana, Grandma, Etc.)
- Last name (one name)
- Number (one player number)
- Design colors (Two colors only/color chart in photos)
